I worked in a Geno's pizzeria in the 80s (near Raven Records, if I recall correctly) that received a lower grade than that. I don't recall the exact score, but I know that our make table temps were much higher than they should have been, the ingredients weren't being rotated, and the final nail in the coffin... our dry storage area had a dirt floor. Needless to say, the store closed not long afterward. To my knowledge, the Pigeon Forge store remains, obviously never having a similarly low health inspection score.

The lowest I recall was the old China Star Buffet. That place failed several times. I'll sort of repurpose a Mitch Hedberg joke here. I ate at China Star one time, and that's the reason I didn't eat there a second time.
